Red Light Therapy: What the Research Supports and What It Doesn't


Red light therapy has gone from dermatology offices to every gym, med spa, and Instagram ad in the San Gabriel Valley. You have probably seen the claims. Burns fat. Reverses aging. Heals injuries. Boosts testosterone. Detoxes your cells.


Some of that is real. A lot of it is not.


We put a Recovery Lounge with red light therapy in our Arcadia studio because the evidence for certain uses is solid. We are also going to tell you exactly where the evidence runs out, because the fastest way to waste money in fitness is to buy the promise instead of the tool.


Here is the honest version.


What red light therapy actually is


The clinical name is photobiomodulation. You sit in front of panels emitting specific wavelengths of red light (roughly 630 to 660 nanometers) and near-infrared light (roughly 810 to 850 nanometers).


The mitochondria in your cells absorb those wavelengths. Mitochondria are the parts of the cell that produce energy. The working theory is that this light exposure improves mitochondrial efficiency, which supports cellular repair and reduces oxidative stress.

That is it. No heat. No UV. No chemicals. It is not a tanning bed, and it is not a sauna, even though the two get sold together constantly.


The mechanism is real and reasonably well mapped. The question is what that mechanism produces in an actual human body, and that is where you have to look at the evidence one claim at a time.


What the research supports


1. Muscle recovery and soreness


This is the use case most relevant to anyone training seriously, and the evidence here is genuinely encouraging.


Multiple systematic reviews and meta-analyses have found that photobiomodulation reduces delayed onset muscle soreness, supports recovery of muscle strength, and lowers biochemical markers of muscle damage such as creatine kinase.


One detail worth knowing: several reviews found the effect is strongest when the light is applied before exercise rather than after. That runs against how most people use it.

The caveats are real. Study protocols vary wildly in wavelength, dose, and device. Results have been less consistent in moderately trained people than in beginners or highly trained athletes. And at least one trial found soreness improved in some muscle groups without improving jump or agility performance.


Translation: it appears to help you feel less beat up and recover strength faster. It is not going to make you a better athlete on its own.


2. Skin quality and collagen


This is arguably the strongest evidence base in the entire field.


Randomized controlled trials using combined red and near-infrared wavelengths have documented measurable increases in collagen density, reductions in wrinkle depth, and improvements in skin texture and elasticity. Most clinical protocols involve roughly 10 to 12 minutes per session, a few times per week, over 8 to 12 weeks.


Realistic expectation: softer fine lines, better texture, modest collagen improvement. Not a facelift. The literature does not support the kind of tightening you would get from a clinical radiofrequency or ultrasound procedure.


3. Temporary pain relief


Where devices are cleared by regulators, temporary relief of minor muscle and joint pain is one of the specific uses on the label. Research on photobiomodulation combined with rehabilitation for conditions like rotator cuff pain has shown benefit for pain and function.

Note the word temporary. It shows up throughout the regulatory language, and it is not a technicality.


4. Certain types of hair loss


Some devices hold specific FDA clearance for androgenetic alopecia, the pattern hair loss that affects both men and women. Trials show measurable improvement in hair density for some users. Results vary considerably by individual.


5. Safety


Red light therapy has an excellent safety profile. A systematic review examining oncologic safety for skin rejuvenation found no clinical trial data linking photobiomodulation to significant adverse events. Side effects, when they occur, are usually minor skin irritation from overuse or an overpowered device.


What the research does not support


Fat loss and weight loss


This is the single most oversold claim in the category, and it deserves a hard line.

Red light therapy does not burn meaningful body fat. It does not create a calorie deficit. It will not move your scale.


What the research shows is narrower: some small controlled trials have found modest reductions in waist, hip, and thigh circumference. Certain devices hold FDA clearance for temporary circumference reduction, sometimes marketed as body contouring. That is a measurement claim, not a weight loss claim, and the effect is temporary and dependent on what you do with your training and nutrition afterward.


If a clinic or a device tells you red light burns hundreds of calories per session or eliminates visceral fat, they are misrepresenting the evidence.


Detoxing, hormone balancing, immune boosting, disease treatment


There is no credible evidence base here. Claims about detoxification, hormone optimization, immune enhancement, or curing chronic disease have not been evaluated or approved by any regulator, and no serious medical authority treats red light as a cure-all.


When one product claims to fix everything, skepticism is the correct response.


Performance enhancement as a standalone


Improved recovery markers are not the same thing as improved performance. Evidence for photobiomodulation adding meaningful strength gains on top of a resistance training program is weak. One review of older adults concluded there is no clear additional benefit to maximal strength from adding it to resistance training.


The training builds the strength. The light supports the recovery around it.


"FDA cleared" does not mean what the ads imply


This is the most useful thing in this entire article, so read it twice.


FDA cleared means a device was reviewed and found substantially equivalent to something already on the market and considered safe. It is a safety baseline.


FDA approved means a specific medical benefit was evaluated and validated.


Almost every red light device you see advertised is cleared, not approved. Clearance says nothing about whether it works for the thing being advertised. A device can be legitimately FDA cleared and still be sold with weight loss claims that the FDA never examined.


Dose matters more than the device


Two specifications determine whether a red light panel does anything at all:

  • Wavelength. Should sit in the 630 to 680 nm red range or the 800 to 850 nm near-infrared range. Combined red and near-infrared appears to outperform either alone.

  • Irradiance. This is the actual power delivered to your skin, measured in mW/cm². If a company advertises "total wattage" instead of irradiance, or does not publish the number at all, that is a warning sign.


There is also a biphasic dose response in this research, which means more is not better. Past a certain point, higher doses of the same wavelength produce worse results, not better ones. Longer sessions do not equal faster results.


This is why the cheap mask you saw online for under a hundred dollars is likely doing nothing. It is not that red light does not work. It is that underpowered devices do not deliver a therapeutic dose.


Who should check with a doctor first


  • Anyone taking photosensitizing medications

  • Anyone pregnant

  • Anyone with active skin cancer or an undiagnosed skin lesion

  • Anyone with a diagnosed eye condition, since eye protection matters


This is a short list and the therapy is well tolerated by most people, but it is worth a conversation with your physician if any of these apply.

If you are choosing between adding a recovery modality and fixing your training consistency, fix the consistency. The light is an accelerator, and accelerators need something to accelerate.


The bottom line


Red light therapy is neither a scam nor a miracle. It is a legitimate tool with a real mechanism, a strong safety profile, and a narrow band of well-supported uses: recovery and soreness, skin quality, temporary pain relief, and certain hair loss.

Everything past that band is marketing.


Use it for what it does. Do not pay for what it doesn't.


Frequently Asked Questions


Does red light therapy help with weight loss? Not meaningfully. Some studies show modest, temporary reductions in body circumference, but red light therapy does not produce real fat loss or lower body weight on its own. Training and nutrition do that.


How often should I use red light therapy? Most clinical protocols run around 10 to 15 minutes per session, roughly three to five times per week, over a period of 8 to 12 weeks. Longer sessions do not produce better results and may produce worse ones.


How long before I see results? Recovery and soreness benefits can show up within days. Skin changes typically require 8 to 12 weeks of consistent use. Anyone promising visible results in a week is selling, not informing.


Is red light therapy safe? For most people, yes. It is non-thermal, non-UV, and has a strong safety record in clinical trials. Check with your doctor if you are pregnant, taking photosensitizing medication, or have a skin or eye condition.


Should I use it before or after my workout? Several reviews found stronger effects when photobiomodulation was applied before exercise. That said, post-workout use is still commonly studied and used. If your goal is soreness reduction, either timing is reasonable to try.


Is a home device as good as a professional panel? Usually not. The variable that matters is irradiance, and most consumer devices either underdeliver or do not publish the number. Professional panels are built to deliver a therapeutic dose across a larger area.
